How to refer

The local clinician/care team complete a referral form ensuring, where possible, all investigations, as shown in the referral form are complete and results available. Please refer to the Pathway and Guidelines if you require further information.

The referral form is e-mailed to national.hcvpmdt@nhs.net
- The National HCV pMDT ODN Co-ordinator will acknowledge receipt of the referral form within 3 working days and may request further information if the form is incomplete. On confirmation that the information is complete the date for the virtual ODN meeting will be confirmed and the local care team will be invited to join the discussion.
After referral
- The co-ordinator will ask referring teams to confirm the name of the person who will be presenting the patient at the meeting and the names of team members who wish to join the virtual meeting
- An MS Teams invite will be sent to all steering group members, referral teams and observers
- An agenda will be distributed confirming referral centres and the number of patients to be discussed.
ODN Patient Review meeting
- Anonymised referral forms will be screen shared, referring team will present their patient
- Chair will invite questions from the attendees and will summarise key discussion points and recommendations
- Treatment dose and duration, will be confirmed by the ODN Pharmacist
Post ODN patient review meeting
- The co-ordinator will, within 5 working days
- prepare an outcome form confirming treatment and recommendations
- E-mail covering letter with outcome form; treatment information leaflet for family and details on Arden and Gem Registry.*
- On receipt of confirmation of treatment, the local team will prepare Blueteq forms to order DAA treatment and patient treatment can be scheduled.
Follow up
- Referral team will manage patient treatment, with support from the specialist centres if required
- In accordance with NHS England requirements the co-ordinator will write to referring teams to capture data at regular intervals to confirm start date of treatment; End of Treatment HCV quantitation; week 12 SVR and SVR 12. Information on side effects and any problems experienced will be requested.
*Mandatory data capture
All referring teams are responsible for submitting data on patients treated through the National HCV pMDT ODN to the NHS England online HCV registry (Arden and Gem).
